Medical Examinations
Pennsylvania Law requires that students attending school receive medical examinations at stated intervals.
Medical examinations are required by Pennsylvania law for those students in grades Kindergarten, 6, and 11.
It is suggested that this examination be completed by your child's medical care provider as they will best know your child's health history and medical needs. A Private Examination form is required to completed and submitted to school. Examinations that are completed up to one year prior to or after your child's current school year may be accepted.
However, if you are unable to have a private examination completed, a basic medical screening may be done by our school doctor onsite at North Clarion, at a date to be determined during the school year. Approximately 2 weeks prior to the school-provided medical screening, a permission form will be sent home with your child. This form is required to be signed and returned along with a completed health history form, prior to the scheduled exam date.
