Dental Examinations
Pennsylvania Law: 28 PA Code 23.3(a)* states that Dental examinations shall be required on original entry into school (typically Kindergarten) and in grades three and seven.
It is suggested that your family dentist complete this examination as they will best know your child's medical history and dental health needs. However, if you prefer, North Clarion does participate in the Smile Mobile Dentist program. Twice a school year (typically October and April), the Smile Mobile Dentist team comes to North Clarion to perform dental screenings and treatments, if necessary.
Notice will be given approximately 2 months prior to the scheduled date of the Mobile Dentist team's visit to the school. A completed student permission/insurance form is required to be signed by a parent/guardian and returned prior to the exam date or alternatively, you may sign up online. If your child is approved to participate, a state-licensed dentist will check your child's mouth and teeth, as well as provide a dental cleaning. Other services such as x-rays, fluoride treatments, sealants, fillings, etc. may be performed right on site, if necessary. A dental report card will be sent home with your child after their visit with the dentist.

Forms are available in the School Health Service's office for exams performed by your family dentist. It is requested that Private Dental Forms be completed and turned in to the school by May 1st of the current school year. Exams that are completed 1 year prior to or after the current school year may be accepted for the current school year.
